2), Aloetin B (3), Aloenin (4), aloesin (5), glycoprotein (6-8), magnesium
lactate
ได้มีการศึกษาฤทธิ์รักษาแผลในกระเพาะอาหารในหนูขาว
(12-18) Parmar และคณะ (17) ได้ศึกษาผลของเจลและยางโดยเหนี่ยวนำหนูให้เป็นแผลในกระเพาะอาหารด้วยวิธีต่างๆ
ได้แก่ การใช้ acetylsalicylic acid, phenylbutazone, reserpine ความเย็น และใช้ cysteamine hydrochloride ทำให้เกิดแผลในลำไส้
พบว่าไม่ได้ผลในการป้องกันการเกิดแผลในกระเพาะอาหาร Koo 1994 (14)
รายงานว่าวุ้นว่านหางจระเข้ไม่สามารถป้องกันการเกิดแผลในกระเพาะอาหาร
เนื่องจากอัลกอฮอล์และความเย็นแต่ช่วยให้หายเร็วขึ้น ซึ่ง Kandil and
Gobran (13) ก็พบผลเช่นกันเดียวกัน
เมื่อให้วุ้นว่านหางจระเข้หลังเกิดแผล 7 วัน
การทดลองต่อมาพบว่าเมื่อให้หนูซึ่งเหนี่ยวนำให้เกิดแผลในกระเพาะอาหารกินวุ้นว่านหางจระเข้วันละ
2 ครั้ง ครั้งละ 5 มก./น.น. ตัว 100 กรัม
พบว่ากลุ่มที่ได้รับว่านหางจระเข้จะไม่มีแผล ส่วนในกลุ่มที่ได้ cortisol
และ cortisol ร่วมกับว่านหางจระเข้ มีแผล
และพบว่าเยื่อเมือกมีความหนาเพิ่มขึ้นในส่วนต้นของต่อมในกระเพาะอาหาร
แสดงว่าวุ้นว่านหางจระเข้จะช่วยลดการเกิดแผล โดยกระตุ้นการสร้างเมือก (18) การทดลองของสิริพันธุ์ (15) พบว่าในหนูที่เหนี่ยวนำให้เกิดแผลโดย
cortisol และให้วุ้นว่านหางจระเข้
สารสกัดว่านหางจระเข้ทั้งใบ เปรียบเทียบกับไซเมติดีน พบว่าวุ้นว่านหางจระเข้ช่วยลดการเกิดแผลได้อย่างมีนัยสำคัญ
ในขณะที่สารสกัดทั้งใบและไซเมติดีน มีผลเล็กน้อย ศิริมา (16) ได้ทดลองในหนูขาวที่ทำให้เกิดแผลด้วย
0.6 N HCl และ acetic acid เมื่อให้วุ้นสด
วุ้นสดที่ทำให้แห้งโดยวิธีแช่แข็ง และวุ้นที่เก็บไว้ในตู้เย็นก่อนนำมาทำแห้ง
พบว่าในขนาด 400 มก./กก./วัน
สามารถป้องกันการเกิดแผลได้ทั้ง 3 ตัวอย่าง
และให้ผลใกล้เคียงกับซูครัลเฟต แสดงว่าป้องกันการเกิดแผลและทำให้แผลหายเร็ว
จากการศึกษาต่อมาพบว่า สารออกฤทธิ์คือ Aloctin A (1), Aloctin B (3) และ polysaccharide (11)
นอกจากการศึกษาในวุ้นแล้วยังมีการศึกษาในสารสกัด
(4,19-22) พบว่าได้ผลในการป้องกันการเกิดแผล
และรักษาแผล และพบว่าสารออกฤทธิ์เป็นสารที่มีน้ำหนักโมเลกุลสูง > 5,000 และ > 50,000 ซึ่งสามารถยับยั้งการเกิดแผลเนื่องจากความเครียด
กรดน้ำส้ม และการทำ ligation of pylorus โดยสารที่มีน้ำหนักโมเลกุล
> 5,000 ให้ผลดีกว่า (20) ต่อมามีการศึกษาพบว่าการรักษาแผลในกระเพาะอาหารของสารสกัดว่านหางจระเข้ไม่เกี่ยวกับการสังเคราะห์
prostaglandin (21)
การศึกษายังพบว่าสารสกัดว่านหางจระเข้มีฤทธิ์ทำให้เกิดแผลในกระเพาะอาหาร
(23) แต่มีผู้พบว่าสารสกัดว่านหางจระเข้มีผลรักษาแผลในกระเพาะอาหาร
และพบว่า magnesium lactate และ aloenin ลดการหลั่งน้ำย่อย (4) การศึกษาต่อมาพบว่า Aloctin
A นอกจากรักษาแผลในกระเพาะอาหารแล้วยังลดกรดและเปบซิน
จึงลดการเกิดแผลเนื่องจากแอสไพริน และอินโดเมธาซิน (2) ซึ่งต่อมามีการศึกษาเพิ่มเติมและพบว่าให้ผลช่วยลดการหลั่งกรดและน้ำย่อย
(22, 24) และสารออกฤทธิ์คือ Aloctin A (2)
สำหรับการทดลองในคนนั้นได้มีการทดลองในยูเครน
โดยนำน้ำวุ้นว่านหางจระเข้มาเตรียมเป็น emulsion ให้รับประทานครั้งละ
2-2.5 fluid drams มาทดลองในผู้ป่วย 12
ราย พบว่าหายทุกราย และนักวิจัยจากยูเครนเชื่อว่าสารออกฤทธิ์ที่สำคัญคือ traumatic
acid ซึ่งเป็นฮอร์โมนที่จะพบในพืชที่มีการทำลายเนื้อเยื่อ
โดยจะปนอยู่กับพวกมิวซิเลจต่างๆ และกระบวนการออกฤทธิ์จะมาจาก
1. ไปจับตัวกับ pepsin
ทำให้ย่อยโปรตีนไม่ได้ แต่จะปล่อยออกมาเมื่อได้รับอาหาร ดังนั้นจึงลดการเกิดแผลในขณะท้องว่าง
2. ยับยั้งการหลั่งกรด
3. เจลมี manuronic และ
glucuronic acid เป็นส่วนประกอบจึงช่วยป้องกันการเกิดแผล (25)
การใช้ว่านหางจระเข้สมานแผลมีมาแต่โบราณ
มีการศึกษาในสัตว์ทดลองต่างๆ เมื่อทาขี้ผึ้งซึ่งมีสารสกัดว่านหางจระเข้เป็นส่วนประกอบ
บนหลังหนูถีบจักรที่ทำให้เกิดแผล (26) Davis และคณะ (27)
ได้ทำการศึกษาผลของว่านหางจระเข้หลายการทดลอง
พบว่าเมื่อนำสารสกัดซึ่งเอา anthraquinone ออกแล้วไปฉีดเข้าใต้ผิวหนังหนูถีบจักร
และหนูขาว ในขนาด 10 มก./กก. พบว่าแผลหายเร็วกว่ากลุ่มควบคุม
การศึกษาพบว่าฤทธิ์การสมานแผลของวุ้นว่านหางจระเข้ใช้ได้ผล
ทั้งการให้สัตว์ซึ่งทำให้เกิดแผลกินในขนาด 100 มก./กก./วัน หรือผสมกับ Eucerin cream 25% ทา (28) ตะกอนที่ได้จากการตกตะกอนสารสกัดเอทานอล (50%)
ทำให้แผลมีขนาดเล็กลง 47.1% แต่ส่วนใสไม่มีผล (29)
mannose-6-phosphate ซึ่งได้จากว่านหางจระเข้ในขนาด 300 มก./กก. ทำให้แผลหายเร็วขึ้น (10)
เมื่อให้ว่านหางจระเข้ 100 และ 300 มก./กก. แก่หนูถีบจักร
จะช่วยยับยั้ง hydrocortisone acetate ที่ทำให้แผลหายช้า
แผลจึงหายเร็วขึ้น (30) และเขาได้จดสิทธิบัตรการใช้ว่านหางจระเข้ในการรักษาแผลเปิด
(31) Udupa และคณะ (32) พบผลเช่นเดียวกัน
เมื่อฉีดวุ้นว่านหางจระเข้เข้าช่องท้องหนูถีบจักร ในขนาด 2 ซีซี/กก. อย่างไรก็ตามมีผู้รายงานว่าไม่ได้ผล (33)
มีผู้จดสิทธิบัตรของขี้ผึ้งที่มีโพลีแซคคาไรด์เป็นส่วนประกอบในเรื่องการรักษาแผลในหนูขาว
(34) การทดลองในหนูขาว โดยทำให้เกิดแผลที่หลังแล้วทาด้วย 2%
mupirocin ointment, 1% clindamycin cream, 1% silver sulfadiazine cream, วุ้นว่านหางจระเข้ และ 1 % silver sulfadiazine ร่วมกับวุ้นว่านหางจระเข้
พบว่ากลุ่มที่ได้รับว่านหางจระเข้แผลหายเร็วที่สุด
และยังช่วยเร่งให้แผลหายเร็วขึ้นเมื่อใช้ร่วมกับ silver sulfadiazine ซึ่งทำให้แผลหายช้า (35) Muller และคณะ (36) พบผลเช่นเดียวกัน และกระบวนการที่ทำให้แผลหายเร็วขึ้นเกิดจากกระบวนการ contraction
(37) และยังมีผู้ศึกษาพบว่าการที่ว่านหางจระเข้ทำให้แผลในหนูขาวหายเร็วขึ้นเนื่องมาจากไปเสริมฤทธิ์
NO หรือจับอนุมูลอิสระจึงทำให้เนื้อบริเวณนั้นไม่ตาย
โดยไปยับยั้ง Thromboxane synthetase แผลจึงหายเร็วขึ้น (38)
และยังพบว่าวุ้นว่านหางจระเข้เมื่อใช้ภายนอกหรือให้หนูขาวกิน
จะเร่งให้แผลหายเร็วขึ้น โดยการไปเร่งการสร้าง collagen (39, 40) และผู้วิจัยกลุ่มเดียวกันยังได้ทดลองให้หนูขาวซึ่งเป็นเบาหวานกิน
พบว่าแผลหายเร็วขึ้น (41) ซึ่งตรงกับที่ Davis และคณะ (42) ได้รายงานว่าเมื่อใช้ว่านหางจระเข้ซึ่งกำจัด
anthraquinone ออกทำให้แผลหายเร็วขึ้น ทั้งในหนูปกติและหนูเบาหวาน
และยังพบว่าวุ้นว่านหางจระเข้ยังช่วยให้แผลหายเร็วขึ้น
และทำให้หลอดเลือดบริเวณแผลที่ขยายกลับเป็นปกติ (43)
นอกจากการศึกษาในหนูถีบจักรและหนูขาวแล้ว
ยังมีการศึกษาในสัตว์ชนิดอื่น ได้แก่ แมว ม้า (44) สุนัข (44,
45) มีผู้ผสมว่านหางจระเข้กับ
Na2S2,O3, CMC, PVP 400, EDTA40 และ buffer และนำไปใช้รักษาแผลในปลา พบว่าได้ผลดี (46) การศึกษาในหมู
พบว่าแผลจะตกสะเก็ดเร็วขึ้น เมื่อใช้วุ้นว่านหางจระเข้ (47) การศึกษาโดยทำให้หูกระต่ายเป็นแผลด้วยความเย็น
พบว่าการใช้ว่านหางจระเข้ร่วมกับ pentoxifylline ทำให้เนื้อเยื่อไม่ตายดีกว่า
เมื่อใช้ pentoxifylline หรือว่านหางจระเข้อย่างเดียว (48)
ไม่เพียงแต่มีผลการทดลองในสัตว์ทดลอง
ยังมีรายงานผลทางคลินิก การนำขี้ผึ้งซึ่งมีว่านหางจระเข้ 50% ไปใช้รักษาแผลถลอก พบว่าอัตราการหายดี (49) มีผู้ทดลองเปรียบเทียบผลของวุ้นว่านหางจระเข้และ
polyethyline oxide พบว่าได้ผลดีกว่าในการรักษาแผลถลอก (50)
แต่รักษาแผลที่ไม่ติดเชื้อได้ผล (51) มีผู้นำไปใช้ในแผลเรื้อรัง
สิว และพบว่าลดการหลุดร่วงของผมด้วย (52) มีการนำมาใช้รักษาแผลภายนอกได้ผลโดยใช้ร่วมกับ
Vitamin E (53) ซึ่งมีรายงานผลดีของว่านหางจระเข้ต่อมาอีกหลายฉบับ
(54-56) การที่แผลหายเร็วขึ้นก็เนื่องจากการที่ผิวหนังเร่งแบ่งตัวเพื่อซ่อมแซมผิว
(57, 58) Thompson (59) ได้นำไปใช้ในผู้ป่วยผ่าตัดจมูก พบว่าทำให้แผลหายเร็วขึ้น
และมีการนำไปใช้รักษาแผลที่เกิดจากการรักษาสิวด้วยวิธีดูดหัวสิว (60) สารสกัดด้วยน้ำเมื่อนำไปใช้หลังการผ่าตัดฟัน พบว่าช่วยให้แผลสมานเร็วขึ้น (61)
การทดลองในผู้ป่วย 31 คนซึ่งเป็นแผลในปาก
พบว่าผู้ป่วย 77% ปวดน้อยลง ผู้ป่วย 80% แผลดีขึ้น (62) และการนำว่านหางจระเข้ไปรักษาแผลที่เกิดจาการถอนฟัน
พบว่าทำให้เกิด alveolar asteitis ซึ่งทำให้แผลหายช้าน้อยกว่ากลุ่มที่ได้รับน้ำเกลือ

อย่างไรก็ตามการนำว่านหางจระเข้มาใช้รักษาแผล
มีผู้รายงานว่าไม่ได้ผล การทดลองในหญิง 21 คน ซึ่งมีแผล
พบว่าแผลหายช้ากว่า standard treatment (64) การนำว่านหางจระเข้ผสม
silicon dioxide และ allantoin ไปรักษาแผลในปากพบว่าไม่ได้ผล
(65) การนำ acemannan ไปใช้รักษาแผลกดทับในผู้ป่วย
30 ราย พบว่าได้ผลพอๆ กับน้ำเกลือ (66)
การศึกษาผลของว่านหางจระเข้ในการเพาะเลี้ยงเนื้อเยื่อพบว่าเร่งการแบ่งตัว
(67) Lectin จากว่านหางจระเข้ทำให้เซลล์ประสานกันได้ดี (68)
ได้มีการนำเอาวุ้นสด
และผลิตภัณฑ์วุ้นที่ทำขายมาทดสอบในการเพาะเลี้ยงเซลล์ผิวหนัง และ fibroblast
ของผิวหนัง พบว่าวุ้นสดให้ผลดีกว่าผลิตภัณฑ์ที่ขายในท้องตลาด
โดยเร่งการเจริญของเซลล์ทั้ง 2 ชนิด (69)
Glycoprotein จากว่านหางจระเข้เร่งการแบ่งตัวของเซลล์ผิวหนัง และไต (6)
และ glycoprotein ยังเร่งการแบ่งตัวของเซลล์ตับ
โดยเร่งการนำเอา [3 H] thymidine ไปใช้สังเคราะห์ DNA
(7, 8) และมีรายงานผลของ aloesin เช่นเดียวกัน
(5) การเร่งการแบ่งตัวของเซลล์ จึงทำให้แผลหายเร็วขึ้น
การศึกษากระบวนการออกฤทธิ์ในการรักษาแผลของว่านหางจระเข้
พบว่ามีส่วนเกี่ยวข้องกับกระบวนการต่างๆ ได้แก่ กระบวนการลดการอักเสบเนื่องจาก Bradykininase
ซึ่งไปทำลาย Bradykinin สารซึ่งทำให้หลอดเลือดขยายตัวและเกิดการอักเสบ
ปวด และ Bradykininase ยังไปเปลี่ยน angiotensin I ไปเป็น angiotensin II ซึ่งทำให้หลอดเลือดหดตัว
จึงทำให้การอักเสบลดลง กระบวนการที่ 2 คือ magnesium
lactate ที่มีในว่านหางจระเข้ จะยับยั้ง histidine
dicarboxylase ไม่ให้เปลี่ยน histidine ไปเป็น
histamine ซึ่งทำให้เกิดการอักเสบ กระบวนการที่ 3 คือ การที่ alloctin A ไปเพิ่มการทำงานของ adenylate
cyclase ใน T-lymphocyte ทำให้ ATP เปลี่ยนเป็น cyclic AMP ซึ่งเร่งการแบ่งตัวของเซลล์ที่ได้รับอันตราย
ซึ่งก็มีผู้พบว่า alloctin A ช่วยในการแบ่งตัวของเซลล์
ดังนั้นจึงช่วยสมานแผล Alloctin A ยังช่วยลดการอักเสบอีกด้วย
นอกจากนี้ยังอาจเป็นไปได้ว่า วุ้นว่านหางจระเข้ช่วยสมานแผลโดยการสร้างโปรตีนจากกรดอะมิโนที่ได้จาก
bradykinin ที่ถูกย่อย และ aloctin A อาจจะช่วยเพิ่มการกำจัดเนื้อเยื้อที่ตาย
โดยกระบวนการกระตุ้นการแบ่งตัวของ leukocytes, macrophages และ
monocytes ซึ่งจะช่วยกำจัดเซลล์ที่ตาย
และในขณะเดียวกันก็เร่งการแบ่งตัวของเซลล์ที่ยังมีชีวิตบริเวณแผลเพื่อมาซ่อมแซมส่วนที่เสียไป

การศึกษาพบว่าว่านหางจระเข้ยับยั้งการทำลายเนื้อเยื่อเนื่องจากการบาดเจ็บต่างๆ
โดยยับยั้ง Thromboxane A2 และรักษาระบบเลือดที่มาหล่อเลี้ยงเนื้อเยื่อบริเวณแผลและหลอดเลือด
จึงทำให้แผลหายเร็วขึ้น (70) ว่านหางจระเข้กระตุ้นการสร้าง glycoaminoglycan
ซึ่งเป็นสิ่งที่ร่างกายสร้างขึ้นมาปิดแผลในขั้นตอนแรกของการสมานแผล
หลังจากนั้นจึงจะสร้างเนื้อเยื่อ granulation, collagen และ elastin
ว่านหางจระเข้ยังไปกระตุ้นการสร้าง hyaluronic acid และ dermatan sulfate และพบว่าปริมาณ glucohydrolase
เพิ่มขึ้น แสดงว่าอัตราการผลัดเนื้อเยื่อ matrix เพิ่มขึ้น จึงช่วยให้แผลหายเร็วขึ้น (71) การศึกษาผลต่อ
collagenase และ metalloproteinase พบว่าส่วนวุ้นยับยั้งการทำงานของ
metalloproteinase ซึ่งไปทำลายเนื้อเยื่อ
จึงลดการทำลายเนื้อเยื่อ แต่ aloin นอกจากยับยั้ง metalloproteinase
แล้วยังยับยั้ง collagenase ซึ่งขับออกมาจากเชื้อ
Clostridium histolyticum ซึ่งเชื้อนี้จะพบในแผลติดเชื้อ
ดังนั้นในกรณีแผลติดเชื้อวุ้นว่านหางจระเข้ไม่มีผลยับยั้งการทำลายของ enzyme
จากเชื้อ (72)
นอกจาก
aloctin A ยังมีสารพวก polysaccharide ได้แก่
acemannan ซึ่งเมื่อฉีดเข้ากล้ามเนื้อ ทำให้แผลหายเร็วขึ้น (73)
และมีผู้จดสิทธิบัตรผลของ polysaccharide ในการสมานแผล
(74) Pickart (75) ได้ทดลองเตรียมอนุพันธุ์กับโลหะได้สารซึ่งมีฤทธิ์สมานแผล
และยังช่วยกระตุ้นการงอกของขนในหนูถีบจักร
ด้วยฤทธิ์สมานแผล
จึงมีผู้นำไปใช้รักษาแผลอักเสบต่างๆ ได้แก่ แผลที่เกิดจากการแพ้ sodium
lauryl sulfate ในผงซักฟอก พบว่าได้ผลดี (76) ใช้ผสมในน้ำยาทำความสะอาดผิวหนังโดยใช้สารสกัดว่านหางจระเข้
(77) ผสมน้ำยาล้างแผล (78) น้ำยารักษาแผลในปาก

3. ฤทธิ์ต้านฮีสตามีน
สารสกัดว่านหางจระเข้สามารถยับยั้งฤทธิ์การหลั่งอีสตามีนของ
mast cell จึงยับยั้งการแพ้ และอักเสบ (80) และการศึกษาต่อมาพบว่าวุ้นว่านหางจระเข้สามารถยับยั้งการหลั่งในกระเพาะอาหารเนื่องจากฮีสตามีน
จึงช่วยลดการเกิดแผลในกระเพาะอาหาร (81) Aloe ulcin จากว่านหางจระเข้ยับยั้ง
amino acid decarboxylase และ histidine
decarboxylase ได้ดีกว่า aloin จึงลดการเกิดฮีสตามีน
(82) และพบว่า magnesium lactate จะยับยั้ง
histidine decarboxylase จึงลดการเกิดฮีสตามีนจากฮีสติดีน (9)
นอกจากนี้ผู้ศึกษาฤทธิ์ของสารอื่นจากว่านหางจระเข้ Aloenin มีฤทธิ์ต้านการหลั่งฮีสตามีนจาก mast cell เนื่องจากสาร
48/80, concanavalin A+ phosphatidylserine และ ionophore
A23187 แต่ไม่ได้ผลในกรณีที่ใช้ antigen กระตุ้น ส่วน Barbaloin มีฤทธิ์แรงกว่าในการต้านฤทธิ์
compound 48/80 และ antigen แต่ให้ฤทธิ์อย่างอ่อนในการต้าน
Con A+ phosphatidylserine และ ionophore A23187 Aloe
emodin ต้านฤทธิ์ compound 48/80 แต่ไม่ต้านฤทธิ์

4. ฤทธิ์ต้านเชื้อ Helicobacter pylori
Helicobacter
pylori เป็นเชื้อแบคทีเรียที่ทำให้เกิดแผลในกระเพาะอาหาร aloe emodin ซึ่งเป็นสาร anthraquinone ที่พบในส่วนยางของว่านหางจระเข้
สามารถยับยั้งการเจริญของ Helicobacter pylori (84)
